Submit an E-Visit

Complete an online questionnaire (E-Visit) through your MyChart account to get a diagnosis and treatment plan from your Specialty Physicians of Illinois provider for common conditions.

Our E-Visit service is available for the following conditions:

      • Cough
      • Diarrhea
      • Heartburn
      • Painful Urination
      • Red Eye
      • Sinus
      • Vaginal Discharge/ Irritation

To use the E-Visit service, you must:

      • Be an existing patient of a participating Specialty Physicians of Illinois provider (Primary Care, Internal Medicine or Pediatric).
      • You must be located in the state of Illinois.

Do not use any apps that use your camera or microphone like Facebook, Zoom, FaceTime, Skype, or Google Hangouts.

Do not allow your device to “Sleep” while waiting for your provider to arrive. If your device goes to “sleep” and when you turn on the screen again, you are prompted to “Resume Session”. Please select continue to avoid any disruptions.
