Accessing MyChart
With MyChart access, you can manage care for you and your loved ones. Here is how to set up access.
There are several methods by which you can access MyChart for yourself or others you care for. Because MyChart provides access to protected health information, we have specific processes and safeguards in place to ensure access is appropriately granted in compliance with state and federal laws.
“I want to access my own MyChart account.”
MyChart login access is available to eligible patients served by Franciscan Health or our partners. To be eligible for your own MyChart login, you must meet at least one of the following conditions:

- An adult (age 18 and older) and able to make your own medical care decisions. For more information on how to do this, review Setting Up a MyChart Login for Yourself (Adult)
- An adolescent (age 12 to 17 years + 364 days) with parent/guardian permission. For more information on how to do this, review Request MyChart Login for Adolescent Patients
Children (age 11 and younger) cannot log into MyChart. However, a parent/guardian can be granted proxy access to the child’s MyChart account as outlined below.
“I want to access someone else’s MyChart account.”
Adults (age 18 and older) can be granted access to another patient’s MyChart account. This is known as Proxy Access. The way this is done depends on the patient’s age and capacity to make their own medical decisions.

- Adults (age 18 or older). If you want access to another adult’s MyChart account, the adult patient must send you an invite to be a proxy. For more information, review Send an Invite to an Adult Proxy

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure access codes, personal ID’s, and passwords. Each person controls their password, and the account cannot be accessed without that password. Further, MyChart uses the latest 1024-bit RSA encryption technology with no caching to automatically encrypt your session with MyChart. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.
You can access MyChart by two methods:
- You can access the website portal from any device using a modern web browser, such as Microsoft Edge, Apple Safari, or Google Chrome.
- You can download the MyChart Mobile application to your Apple or Android device (smartphone or tablet).

Go to the Password Reset page to reset your password online.
Go to the Recover Your MyChart Username page to reset your password online. After correctly answering your security questions, your username will be sent to the email address you provided when you signed up for MyChart.
To protect your health information, your MyChart account will lock after five incorrect login attempts. To have your account unlocked and for assistance resetting your password, please call the MyChart Support Center at 1-877-900-5741.
Yes, you can. This is called proxy access and allows you (as a proxy) to log in to your personal MyChart account and then connect to information regarding your family member.

No, due to the sensitive nature of medical information, each adult must establish their own MyChart account. You may request proxy access to each other’s record, but you will need to log in to your own account to view either your own record or your spouse’s record.

To revoke access to your MyChart, log into MyChart and from the left menu, go to the My Family’s Records section and select Family Access Settings. Finally, select the name of the proxy you no longer want to have access and click the revoke access button.
