Your Medical Record

MyChart Care Companion is an interactive plan of care delivered to patients through MyChart. Care Companion delivers task reminders, analyzes data provided by patients and connected devices, and orchestrates changes to the care plan and escalations to help patients, their caregivers, and care managers stay on top of a patient’s care.

In this topic, we will be using Menstrual Tracking care plan as an example.

If someone has invited you to have proxy access to their MyChart account, you will receive an email with guidance on how to accept.

In addition to viewing your medical information in MyChart, you can also request a formal copy of your medical records.  Formal medical record copies can be used to provide documentation for insurance and legal purposes.  These documents are delivered to you securely via MyChart.

You will generally receive an answer within 1-3 business days. Please note that MyChart should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.

Your MyChart information comes directly from your electronic medical record at your doctor’s office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

Tests of a very sensitive nature may not be released to MyChart until after your care team has shared these results with you.

Most test results are now available to you in MyChart at the same time they are delivered to your physician. This means you might review your test results before your Provider has had the chance to see them. As always, if you have questions about any particular test, please contact your Provider’s office.

Here are instructions on viewing results:

Mychart will send you an email notification when you have new information available in MyChart, such as a test result or an upcoming appointment. Also, if you ever forget your MyChart username, it can be emailed to you after confirming the answers to your security questions.
