Request A Copy of Your Medical Records
NOTE: This feature is currently available to Franciscan Health and Specialty Physicians of Illinois patients. For more options to obtain your medical records, please review the information on the Resource tab.
In addition to viewing your medical information in MyChart, you can also request a formal copy of your medical records. Formal medical record copies can be used to provide documentation for insurance and legal purposes. These documents are delivered to you securely via MyChart.

Log into MyChart
See login options: MyChart Login

In the menu, search for sharing hub

Select Request Formal Copy of Health Record

Answer the questions and select continue

Select the checkbox to I agree and click Send Request

The system will bring you to the Document Center to review the Statuses of your Medical Records. Once complete, you will be able to download and view your documents.
