Patient Satisfaction Surveys (by Medallia)
NOTE: This applies only to Franciscan Health and Specialty Physicians of Illinois patients. Other partners may conduct surveys as well; however, those processes are not included in this topic.
As a patient, you may receive a survey after using our services. Participation in the survey requires a valid mailing address, email, or mobile phone number.
The Medallia Patient Satisfaction Survey is a tool used by Franciscan Health to collect feedback from patients about their wellness journey. The survey focuses on key aspects of patient care, identifying areas of excellence and opportunities for improvement in the healthcare experience. Your valuable feedback helps us better understand your needs and expectations, fostering a partnership of trust.

Electronic Survey Link via Email
Step 1: You will receive an email invitation with the Subject line, ” Your Franciscan Health team would like to hear from you!
Step 2: Open the email. The email will greet you with ” Thank you for choosing Franciscan Health, [the location and department you visited]; it is our honor to care for you, and we’d love to hear feedback!
Step 3: Start the survey by choosing a score for how likely you would recommend us, then follow the survey prompts to finish.
Step 4: A reminder email will be sent within five days if the survey is not completed.
Step 5: Opt-Out- To unsubscribe from future Medallia surveys for Franciscan Health, click the link at the bottom of the email invitation.

Electronic Survey Link via Text Message
Step 1: First you will receive an invitation via text message with the Subject line, ” [Your Name] we want to hear your feedback regarding your visit to [location and department you visited] and the date of your visit.
Step 2: Second, Click the link to start a 2-minute survey: The survey link will follow.
Step 3: Start the survey by choosing a score for how likely you would recommend us, then follow the survey prompts to finish.
Step 4: Unsubscribe- You have the ability to unsubscribe from survey invitations by sending a reply of “STOP.”
Step 5: A confirmation of your request will immediately follow” You unsubscribed from SMS patient surveys.

Paper Survey via USPS
Step 1: You will receive a paper survey in the mail.
Step 2: Open the envelope and you will have a cover letter from a Franciscan Health leader and a paper survey.
Step 3: Answer all the questions by checking the box to the left of your answer.
Step 4: At times, you will be instructed to “skip over” some questions; when this happens you will see an arrow with a note that tells you what question to answer next.
Step 5: Please answer the questions only for the date included in the cover letter. Please do not include any other procedure dates in your responses.
